Our services for the professional

Case management:

We have worked for several years with a large number of different case management companies. We enjoy working as part of a collaborative team enabling an individual to return to important life roles, both old and new. We have supported people in many different activities most importantly at home but also including returning to work, driving, leisure pursuits i.e. horse riding, cycling, golf, swimming, fishing, sailing.

We provide education and guidance to support workers to promote the rehabilitation process and in time reduce dependence on therapists.

We carry out in-depth risk assessments, provide timely reports and liaise regularly with other team members and external agencies, aiming for the very best for the individual. We feel that communicating well is one of our strengths.

Provision of supervision to other Physiotherapists and Occupational Therapists.

Care homes:

For many years we have provided input into local care homes which includes mobility and functional transfer assessments,  environmental assessments, equipment recommendations. We work with staff to meet the changing needs of residents to enable them to stay in that setting for as long as possible. We also provide input for residents during periods of respite.

We piloted and developed a falls prevention programme which is now carried out on a weekly basis within one care home, and includes strengthening, balance, improving confidence and education. This was positively reviewed by the home's CQC inspection. We have assisted a care home to analyse their residents falls to provide a safer environment and provide training to staff on how to analyse falls and prevention.
